Passport Application Information

Applicants for Ghanaian passports must be citizens of Ghana by Birth, Naturalization, Registration, Adoption, or by any Enactment. Documentary evidence must be provided to support any such claim.

A Ghanaian passport is the only reliable and universally accepted travel and identification document for Ghanaians traveling abroad.

Issued to Ghanaian citizens only, a Ghanaian passport reflects the identity of the applicant as determined by documentary evidence of citizenship and documents to support identity.

 

  • Create an account and complete the online application form.
  • Select an appointment date and time.
  • Upload a recent photo during the online application
  • Make and confirm your online payment.
  • Proceed to the Mission with your printed passport form and any other relevant documents for in- person vetting appointment.

 

  1. PHOTO UPLOAD
    • Must be in PDF or JPEG format against a white background.
    • Full face, forehead and ears visible.
    • No eyewear, large earrings, or decorative headgear is allowed.
    •  Religious headwear is permitted.
  2. GUARANTORS

    Applicants must provide two (2) guarantors who:

    • Are 18 years or older.
    • Have known the applicant for at least two (2) years.
    • May be relatives.
    • Agree to the use of their name and contact information for the application.
  3. WITNESS

    Applicants must provide one (1) witness. The witness must:

    • Sign the application form.
    • Must not be the same person as the Guarantor (s).
    • Be a professional in one of the following fields: Armed Forces, Prison Service, Police Service, Civil/Public Service, Registered Medical Practitioner, Solicitor/Barrister, Head of Educational Institution, any other registered recognised professional or Ghana National Association Chairman/ Secretary.
    • Be available to be contacted by the Mission if needed.
    • Agree to their name and contact information being used for the application.
    • Have known the applicant for at least two (2) years.
    • Be a resident of The Netherlands, ideally a Ghanaian.
    • Applicants may use a registered professional in The Netherlands in the event that applicants are unable to provide a witness who is Ghanaian.
  4. PASSPORT ACQUISITION FOR INFANTS AND CHILDREN BELOW EIGHTEEN (18) YEARS
    • Duly completed and printed online application form;
    • Original Birth Certificate for first time applicants
    • Consent form from both parents.
    • Photo ID of both parents.
  5. RENEWAL, EXHAUSTED PAGES OR EXPIRED PASSPORT (BIOMETRIC)
    • Duly completed and printed online application form;
    • The exhausted or expired passport;
    • Consent form and bio data pages of parents Ghanaian passport for children below eighteen (18) years;
    • However, applicants holding the black book (machine readable) must provide original copy of their Birth Certificate.
  6. MISSING, LOST OR STOLEN PASSPORTS
    • Copy of the bio-data page of the lost passport;
    • Police Report of lost passport.
    • Original Birth Certificate authenticated by the Judicial Service of Ghana and the Ministry of Foreign Affairs of Ghana).
    • Affidavit on the missing passport by the applicant.
    • Any other relevant supporting documents.
  7. REQUIREMENTS FOR DAMAGED PASSPORTS
    • The damaged passport booklet;
    • Affidavit on the damaged passport;
    • Any other relevant supporting documents.
  8. REQUIREMENTS FOR CHANGE OF NAME AND CHANGE OF DATE OF BIRTH
    • Original Marriage Certificate;
    • Duly authenticated documents such as Gazette/Affidavit, with Statutory Declarations
  • Printed passport application form (signed, witnessed and stamped by a recognised professional).
  • Original documents such as expired and damaged passport booklet;
  • Birth Certificates, police report and affidavits must always be presented.
  • Copies must be certified as true by a recognised notary public.
  • The Mission has the right to reject or request additional documentation during the in-person vetting appointment.
 

KINDLY NOTE THE FOLLOWING:

  • Do not wear white dress/shirt for your appointment.
  • Physical passport photos are not required.
  • Children below the age of eighteen (18) years need parental (father and mother) consent be it a new application or renewal.
  • In the absence or death of one (1) parent, the following documents are required;
    • Divorce certificate
    • Death certificate
    • Document proving sole custody of the child from the court of law

  • Payment is made online only using credit card.
  • Cash payments are not accepted under any circumstances.

In line with existing statutory directives, applicants are informed that all Consular fees are non-refundable once paid. This policy includes payments made for incomplete and/or withdrawn applications.
